  1. Program Director: Administrative Position

Duties and Responsibilities: Will oversee project implementation and personnel within Article 31 Clinic, including hiring; core services; adaptation of protocols and procedures; CQI and staff training; and leading Mobile Crisis Response. Program Director will develop comprehensive treatment models that includes direct services, referrals and case management of mental health and substance use.

Special Requirements: Mental Health Professional with at least 3-5 years’ experience in Article 31, supervision, and grant work. Experience with CCBHC grants is a plus!

  1. Social Work Supervisor: (PT/FT)

Duties and Responsibilities: Provide weekly supervision to interns and staff psychotherapists, supervise crisis management and provide administrative coverage of clinic, complete required documentation including intern evaluations, reviewing, and co-signing notes, TPs and assessments. Ensure compliance with Office of Mental Health (OMH) standards.

Special Requirements: part time, two years of supervisory experience is preferred. Must be LCSW licensed in the State of New York. Good organizational, teamwork and communicate skills needed and must have strong clinical group, family, and individual intervention skills.

  1. Part Time Clinician:

Current LCSW, LMHC, PsyD or PhD with 10+ years of experience in individual and / or family psychotherapy. Specialty training / competency in one of the following areas is a plus: Couples Counseling, Trauma, IFS, Somatic, DBT, Play Therapy. Evening and Sunday availability preferred.  Offering very flexible hours and private practice like structure and compensation.

  1. Evaluator: heavy experience with data.

Duties and Responsibilities: Will be responsible to oversee all data processes including integration of data points into data collection procedures and internal data systems. Orientation of staff to new protocols. S/he will monitor progress toward goals and objectives, continually evaluate/assess program performance qualitatively and quantitatively; collaborate with key personnel on project to develop data procedures directly related to direct services, referrals and case management of mental health and substance use service model.

  1. Care Coordinator:

Duties and Responsibilities: The Care Coordinator/Case Manager will be responsible for ensuring holistic care coordination and tracking of patient care to achieve treatment goals. Specifically, s/he will ensure that consumers receive clinical and support services including therapy, case management, family counseling, peer support, appropriate referrals and whatever additional services are needed to support recovery and independence (housing, food, transportation etc.). S/he will also oversee all workflows and care navigation processes including consents, as well as care navigation and data exchange with community partners.

Project will include ability to implement this approach of service delivery within a comprehensive treatment model that includes direct services, referrals and case management of mental health and substance use.

Special Requirements: Professional with at least 2-3 years of experience in case management and clinic work, preferred.

  1. Case Manager:

Full-time or part time case manager, either MSW or MHC, must demonstrate a strong ability to assess family needs, develop treatment plan, offer budget guidance, and evaluate progress from a holistic perspective. Evening hours sometimes needed one night a week.

  1. Psychiatric Nurse Practitioner:
  • A Nurse Practitioner degree with specialized training or experience in Psychiatry. A current license to practice as a nurse practitioner in the State of New York. A current DEA registration for the prescribing of controlled substances. ANCC certification or pending application is required. NYS OPRA (Ordering/Prescribing/Referring/Attending) status approved or application pending required and a Health Commerce Account in place by the onset of employment. The former is needed to prescribe to clients with Medicaid, the latter is needed to adhere to NYS I-STOP law.
  • Provide health/behavioral health assessment (including differential diagnoses, prognosis), treatment recommendations and treatment planning for clients across program types.
  • Provide medication management for clients on medication and intermittent reassessment (as needed) for those assessed (but not on medication). Medication provision includes the direct delivery of injectables.
  • Provide counseling, brief therapy, psychoeducation about health/behavioral health and other care for clients as planned by the treatment team and to support the treatment plan of the client. In most cases psychotherapy is provided by other clinicians. On occasion, when in the best interest of the client and when consistent with program staffing, these services are also provided by the Psychiatric Nurse Practitioner.
  • Participate in case reviews and contribute to treatment planning goal setting.
  • Provide consultations to non-medical staff on complex cases
  • Communicate with health care providers outside of the agency and interpretation of health results for client care planning.
  • Monitor client progress; recommend discharge and aftercare planning.
  • Participate in practice improvement projects that benefit the clients and program.
  • Sign and abide by the provisions of at least one Collaborative Practice Agreement with a psychiatrist employed by SBH.
  1. Epidemiologist:

Duties and Responsibilities: The Epidemiologist will assist with and review Community Needs Assessment design, proposed survey instruments and results.

Special Requirements: 3-5 years’ experience in related work preferred.

  1. Needs Assessment Consultant:

Duties and Responsibilities: Will design and conduct a Community Needs Assessment (CNA) and ensure compliance and highest quality of care via utilization reviews and risk assessment.

Special Requirements: 3-5 years’ experience in related work preferred. Understanding of compliance, a plus.
